In case we can charge your insurance, your co-pay or co-insurance will be due at the time of the visit.

 

If we cannot charge your insurance following fees apply:

 

Initial treatment’s fee is $ 120

Follow-up treatment is  $ 100

 

I offer discounts if you need frequent treatments in a shorter time frame.

 

If you are not able to come to your appointment, please call or text to cancel or reschedule at least 12 hours prior. Otherwise a fee of $50 will apply.

 

Returned checks will carry a service charge of $ 30.
